How to Apply

To apply to be an assistant, you will need to complete the application and provide at least two professional references and two personal references.  There are also some additional questions that you may choose to answer.

Here are the forms you need.  Each form is available in two formats.  Use .doc format if you plan to use Microsoft Word or another compatible word processor (such as Google Documents) to complete the forms. You may want to use .pdf format if you plan to just print out the forms.
